SUPPORT WORKER – OUTREACH - (MHAPD)
• Nunawading location
• Part Time position, 30.4 hours per week
• Salary range between $43,588 - $45,280 p.a. pro-rata
• Tax free salary packaging up to $16,050
Mental Health Accommodation Pathway at Discharge (MHAPD) is a service operating within Mind in the metropolitan central east to support and access accommodation for patients exiting clinical mental health facilities who are at risk of homelessness. MHAPD is located in Nunawading.
The Support Worker - Outreach will identify appropriate discharge pathways for patients exiting from the acute psychiatric inpatient unit (Upton House), and other identified clinical mental health facilities. This position will also provide support to patients experiencing recurring homelessness.
